Admission Credentials Transfer Students Foreign Students Old/Returning Students

High school graduates enrolling for the first time in college should submit the following:

  • a. Original and authenticated Form 138_A, or its equivalent.
  • b. Original copy of the certificate of good moral character duly signed by the school head of the school last attended.
  • c. Result of the CSC Admission Test.
  • d. Original/Authenticated copy of birth certificate
  • e. Medical Health examination record
  • f. Three (3) copies of I.D. pictures

Transferees:

  • a. Honorable Dismissal for transferees
  • b. Transcript of Records for evaluation purposes
  • c. Three (3) copies of I.D. pictures
  • d. Original/Authenticated copy of birth certificate

Graduate:

  • a. Original Transcript of Records for the degree course graduated.
  • b. Special Order for graduation or its equivalent.
  • c. Certification of passing the Graduate Admission Test.
  • d. Honorable dismissal and Transcript of Records for evaluation for new entrants and transferees.
